Twins at 40 and a Dream Fulfilled

At 40 years of age, Khun Jarumas fulfilled her dream of having twins.

After struggling to conceive naturally for years and with concern creeping into her mind around her age, she feared the worst, as post-40 was, in her mind, the end of her fertile years. As a last ditch effort, she decided to consult Dr. Ton, who reassured her that IVF/ICSI had helped other couples her age (or older) conceive despite certain pre-existing conditions (age included).

With essentially nothing left to lose, she decided to start the IVF process immediately during her next menstrual cycle, receiving 10 days of ovarian stimulation. Dr. Ton monitored her progress through ultrasounds to assess follicle development, then proceeded with egg retrieval with the embryos cultured to the blastocyst stage.

Due to the patient’s age, Dr. Ton was very adamant about chromosomal testing of the embryos, which could increase the likelihood of chromosomal abnormalities, such as Down syndrome. Post-chromosomal testing resulted in two healthy embryos, and the patient, hoping for twins, discussed the possibility with Dr. Ton about the possibility of implanting both embryos together. After an evaluation of her health and a long discussion of the future risks involved in carrying twins, Dr. Ton agreed to her request and after 14 days post-transfer, a home pregnancy test revealed two lines.

The patient promptly scheduled an appointment with Dr. Ton for an hCG test, which confirmed high hormone levels, confirming the results of her home pregnancy test. Three days later, another surprise awaited her, as an ultrasound revealed two gestational sacs, and Dr. Ton confirmed that the pregnancy was indeed twins!

Even in these modern times, nature knows best. If a woman's BMI falls below 19, the body senses famine and ovulation is switched off to prevent the risk of having a baby with malnutrition. Excessive exercise can reduce body fat and increase muscle mass to a point where periods cease for the same reason. Risk of miscarriage is also increased in women with a low BMI.
