Get to know ICSI
ICSI —or intracytoplasmic sperm injection— is a special procedure under the umbrella of assisted reproductive technology that increases the chance of a successful pregnancy. It involves the use of a machine that assists in fertilization without waiting for sperm to find the egg on its own.
With ICSI, the one strongest sperm will be injected directly into the egg. Once the egg is fertilized, becoming an embryo, it will continue to develop in the laboratory. This embryo will continue to grow in a special solution until it becomes a blastocyst, where it will then be transferred back to the uterus to continuing growing in pregnancy.
The ICSI machine is the latest in assisted reproductive technologies and the fertilization rate with ICSI, when compared with IVF, increases by 15%. There are only a few of these machines in Thailand and are very expensive.

The first payment
- Medication for ovarian stimulation in the amount of 3,000 IU (if more medication is needed, additional costs will be incurred as follows: 1,500 IU at 37,500 baht and 3,000 IU at 75,000 baht)

Payment Method
Beyond IVF’s ICSI package can be paid in three installments, based on where you are in the IVF process:
- 1st Installment: payable at the start of egg stimulation
- 2nd Installment: payable prior to egg collection
- 3rd Installment: payable prior to each embryo transfer
Payments are split into installments to ensure that, for example, if the process can’t move past the egg stimulation step due to poor response to the stimulation medication, you’ll only be paying for the first step (whereas other IVF centers may charge in full, up-front).
The ICSI process at Beyond IVF (Step-by-step)
Step one: Doctor consultation
The doctor will explain the procedure and preparation and set up the treatment plan. (Please see the doctor before your period.)
Step two: Ovarian stimulation
The doctor will check your hormone levels and carry out an ultrasound to start. You will receive an ovarian stimulation injection on the second or third day of your cycle. The doctor will then schedule ultrasounds and blood tests to monitor the growth of your eggs and check your hormone levels. This process takes approximately 10 days.
Step three: Egg and sperm collection
Once the eggs have reached the desired sizes, the doctor will remove them from the ovaries. The procedure is done under sedation and a needle is inserted through the vagina into the ovaries to collect the eggs. This procedure takes 20-30 minutes. Sperm also needs to be collected and only the best quality sperm are kept to be used in the fertilization process in the laboratory.
Step four: Embryo development
Embryos are kept in the laboratory until they become six- to eight-cell embryos and then on to strong blastocysts.
Step five: Embryo transfer
Once ready, the doctor and patients will select an embryo to be transferred into the uterus. This is done through the vagina and does not require anesthesia.
Step six: Pregnancy test
The doctor will schedule blood tests every three days after embryo transfer to check hormone levels. Pregnancy can be tested for 14 days after the transfer.

Two transfers, successful the second time with twins with ICSI with Dr. Phunsak at Beyond IVF
This patient had never been treated for infertility, but at 37 years old, they had tried to get pregnant for more than a year and felt confident that could not get pregnant on their own. They began researching their options regarding treatments for infertility and found Beyond IVF. They had already looked at all the options available to them for getting pregnant, whether it be IUI, insemination, and IVF, but wanted the option with the highest chances of success where they could observe every step in the process and feel well cared for during the process. Unable to get pregnant naturally, successful ICSI treatment with Dr. Phunsak at Beyond IVF
Another patient that should have been able to get pregnant naturally as they weren’t at the age we normally start to think about infertility and they had already had a child. Even that isn’t a guarantee that you can get pregnant on your own again. The older you get, your uterus, ovaries, and reproductive hormones deteriorate naturally over time. This patient tried for two years to get pregnant and would ultimately be considered as experiencing infertility.
The patient discovered Beyond IVF and read reviews from Dr. Phunsak’s successful patients, which drew them to consult with the doctor and undergo a physical evaluation before requesting to be treated with ICSI as well as NGS for chromosomal screening of their embryos to check for completeness of the 23 pairs of chromosomes and the XY chromosomes. After embryo transfer Dr. Phunsak used the Beyond Boost technique (a treatment at Beyond IVF) to balance the patient’s hormones to increase the chances of a successful pregnancy. After 18 days the patient returned for an ultrasound and discovered they were pregnant with a baby boy! They returned home with a full heart.
Previous tubal ligation, multiple failed embryo transfers after ICSI elsewhere, finally successful ICSI the first time with Dr. Phunsak at Beyond IVF
This patient is 37 years old and underwent tubal ligation after giving birth to their last child, but then decided they would like to have another baby. They chose to us IVF/ICSI rather than reverse the tubal ligation because the chances of a successful natural pregnancy were lower than with assisted reproductive technologies. So the patient decided on ICSI at another fertility clinic and underwent several embryo transfers, but was only able to become pregnant one time, which ended in a miscarriage. The patient then wanted to try a different institution with higher success rates, bringing them to Beyond IVF where there were many successful cases of patients of the same age and in the same situation.
Since it had been longer than a year since their last treatment, Dr. Phunsak requested a thorough physical evaluation of both partners to find the cause of the failed ICSI procedures. While the female partner had normal results, the male partner’s sperm was not normal, and that may be why they were only able to achieve few embryos that were of low quality. Dr. Phunsak informed the patient that they could try ICSI again along with NGS to screen embryos before they are transferred.
Ultimately the one golden embryo was able to achieve a successful pregnancy. The patient said, “Without Beyond IVF, we would have kept trying naturally and likely continue to miscarry due to chromosomal issues.
