This case involves a 39-year-old woman who had previously undergone three unsuccessful ICSI treatments at other fertility centers. Determined to find a solution, her husband began researching fertility specialists with a high success rate for women close to 40 years old. That’s when they discovered Dr. Ton Poonsak at Beyond IVF, who had successfully helped many patients of a similar age conceive.

After consulting with Dr. Ton, they realized that the previous treatments lacked proper documentation and comprehensive testing. Dr. Ton recommended a full medical evaluation for both partners to pinpoint the cause of infertility and create a personalized treatment plan. Although their overall health appeared normal, their past ICSI cycles had only cultured embryos to the 3-day stage without genetic screening before embryo transfer—one of the key reasons for past failures.

Dr. Ton proposed another ICSI cycle using the Blastocyst Culture technique, which allows embryos to develop to the 5-day stage (Blastocyst) before transfer. After 10 days of ovarian stimulation, the patient underwent egg retrieval, resulting in 25 mature eggs, out of which 10 successfully developed into blastocysts. To improve the chances of success, Dr. Ton recommended Next-Generation Sequencing (NGS) for all embryos to screen for chromosomal abnormalities, preventing genetic disorders such as Down syndrome, cleft lip, and developmental delays.

Following NGS screening, only 4 embryos were genetically normal, which the patient considered a great success. Dr. Ton carefully selected the highest-quality embryo for transfer to maximize the chances of implantation.

After 18 days, an ultrasound confirmed a pregnancy sac, bringing immense joy to the patient. However, at one month, unexpected complications arose—the embryo stopped developing due to hormonal imbalances. A thorough examination confirmed that the patient’s body wasn’t producing enough hormones to sustain the pregnancy.

Despite the setback, the patient remained determined to continue treatment. Dr. Ton recommended a three-month break to allow her body to recover while taking medications to regulate her hormones. After re-evaluation, her hormone levels had significantly improved, making her an ideal candidate for another embryo transfer.

With only two embryos remaining, this transfer felt like a final attempt. This time, her body was fully prepared for implantation. Just 15 days after transfer, a blood test revealed HCG levels exceeding 1,000 mIU/ml, and an ultrasound clearly showed a healthy pregnancy sac.

The patient was overwhelmed with joy—despite previous failures, she finally achieved a successful pregnancy on her second attempt at Beyond IVF. Thanks to Dr. Ton’s expertise, she was able to bring her baby home happily.
