At the age of 38, with the last "golden embryo," pregnancy was successfully achieved through the ICSI method by Dr. Ton at Beyond IVF

This case involves a 38 year old patient who already had two children. She had always believed that she wouldn’t face fertility issues because she had previously conceived, but after nearly 2 years of trying naturally for another child without success, she was officially diagnosed with infertility. She decided to seek treatment and found reviews of successful cases at Beyond IVF, which led her to consult Dr. Ton Poon-sak.

After a thorough examination, it was discovered that the patient’s AMH (Ovarian Reserve Hormone) was less than 2 ng/mL, which was significantly low. This made natural conception or IUI (Intrauterine Insemination) less likely, as her eggs might not be of sufficient quality to fertilize and implant successfully. The success rate would be lower with these methods compared to ICSI (Intracytoplasmic Sperm Injection), where the quality of eggs is assessed before fertilization, ensuring a higher chance of success.

The patient decided to proceed with ICSI to reduce the risk of using poor-quality eggs. After starting stimulation for egg retrieval, the ovarian response was not as expected, leading to only 11 eggs being retrieved. However, only a few eggs were of usable quality:

  • 5 eggs in the M II stage (mature eggs, ready for fertilization)

  • 4 eggs in the M I stage (immature eggs, not yet ready for fertilization)

  • 2 eggs in the GV stage (very immature eggs)

After the eggs were cultured, 2 embryos reached the blastocyst stage (5 days old), and 1 embryo passed the chromosomal screening. Luckily, this "golden embryo" was of good quality, so Dr. Ton carefully planned the treatment to prepare for the embryo transfer.

Dr. Ton recommended the patient to undergo body conditioning and nutritional supplements (a special technique from Beyond IVF) for almost 2 months to ensure the best possible outcome. After the embryo transfer, the patient followed the advice given, both before and after the transfer. 18 days later, during the ultrasound to check for pregnancy, Dr. Ton confirmed: "You’re definitely pregnant," as a small embryo was visible in the mother’s uterus.

The patient was treated with ICSI IVF, with additional techniques including:

  • ICSI (using a special camera to inject sperm directly into the egg to increase success rates)

  • NGS (Next-Generation Sequencing, chromosomal screening of the embryos)
