Low ovarian reserve, a cause of infertility, successfully treated with ICSI by Dr. Ton, leading to a successful pregnancy
This patient had been trying to conceive naturally for many years but started to worry because many of their relatives and people around them also faced fertility issues. This concern led them to seek a medical evaluation at Beyond IVF.
The test results showed that the male partner was completely normal. However, when Dr. Ton performed an internal ultrasound on the female partner to examine her ovaries, he found only five antral follicles combined from both ovaries, which is significantly lower than the normal range for her age. This condition could make natural conception even more difficult.
Despite this, there was a silver lining. While the antral follicle count (AFC) was low, her AMH (Anti-Mullerian Hormone) levels—which indicate ovarian function—were still within the normal range, meaning her egg quality remained good. Given this, Dr. Ton advised that she could start fertility treatment immediately. However, he recommended the ICSI (Intracytoplasmic Sperm Injection) procedure, as it would be more effective than IUI (Intrauterine Insemination) for her case. Since both ovaries required stimulation, using injectable medications would be more effective than oral stimulation drugs. If her ovaries responded well, they could potentially retrieve more eggs than the five follicles seen on the ultrasound.
Starting the ICSI Journey
After discussing the options, the couple decided to proceed with ICSI to maximize their chances of conception as quickly as possible. On the second day of her menstrual cycle, the patient started the treatment process.
As expected, her ovaries responded well to the stimulation medication. The ultrasound revealed a total of 17 mature eggs, significantly more than initially anticipated. The retrieved eggs were then fertilized and developed into Blastocyst-stage embryos, which were screened using NGS (Next-Generation Sequencing) to check for chromosomal abnormalities.
The results showed that five embryos were chromosomally normal and viable for transfer. This case proved that despite having a low antral follicle count, as long as AMH levels are within the normal range, there is still a high chance of a successful pregnancy.
Dr. Ton selected the highest-quality embryo for transfer, and the patient successfully conceived.
